Patient Attribution Management

One of the many ways THN provides optimal care for our patients is through attribution. Typically, patients select their Primary Care Provider (PCP) during open enrollment or the Insurer assigns a PCP for patients through claims filed by a Provider. This process is called attribution and it is a key program methodology used to identify the beneficiaries associated with THN and defines the population for which THN is held accountable. However, there are times when patients need their attribution changed for various reasons. Our Quality team manages these attribution correction requests and works with the practices and the Insurance plans to ensure each patient is attributed to the Provider they are seeing for primary care.
