The sixth annual Case in Point Platinum Awards were presented May 7 in Baltimore, Maryland. The success of two Cone Health employee wellness programs has earned Triad HealthCare Network Care Management a top national award.
THN Care Management received a 2015 Case In Point Platinum Award in the category of wellness/prevention for facilitation and management of the Link to Wellness and Healthy Pregnancy programs. Sponsored by Dorland Health, the awards were presented in 26 categories to recognize the most successful programs and people in health care coordination and case management in the United States.
The Link to Wellness and Healthy Pregnancy programs are available to employees and family members covered by the Cone Health insurance plan. Link to Wellness is available for those who are at higher risk for serious health problems due to asthma, diabetes and pre-diabetes, high blood pressure, high cholesterol or heart disease. The Healthy Pregnancy program focuses on educating employees and family members during their pregnancies for healthier outcomes.
Participants in the programs get help managing their health conditions from RN care coordinators, pharmacists, dieticians, certified childbirth educators, and licensed clinical social workers. As a benefit, they also receive certain free medications, free health education and incentives for completing classes.
The THN Care Management team has been serving and caring for Cone Health employees and family members since 2006, and this commitment is paying off. Link to Wellness participants, on average, have shorter and fewer hospital admissions and their health care costs have declined overall. The Healthy Pregnancy program has helped lead to a decrease in premature deliveries and NICU admissions among participants.
Rhonda Rumple, RN, MSN, BSN, CCM, the director of THN Care Management says “We are excited and honored to win this award. The Link to Wellness program has helped support our health plan members who were faced with many different barriers in managing their diabetes. Many are better connected with their medical providers. This has resulted in reduced risk of developing other conditions that could have had a negative impact on them and their families they love. We’ve taken a giant step forward in helping these members manage their conditions, feel better and live healthier lives.”
Founded in 2010, Triad Healthcare Network is a consortium of more than 1,200 Cone Health and independent community providers who work together to improve the health of people in Alamance, Guilford, Randolph and Rockingham counties. THN Care Management focuses on wellness, providing education, coordination and other support to both providers and high-risk patients.
